Adult Behavioral Home Health Coordinator (Case Manager)

AMHC has an immediate opening for a Full-Time Behavioral Health Home Coordinator-Community Integration Specialist (BHH Coordinator) in our Ellsworth area.
The BHH Coordinator is responsible for assisting clients with long-term psychiatric disabilities to attain the skills and resources necessary to successfully function in the community.
The person will emphasize is placed on holistic care using 8 dimensions of wellness:
Emotional, Financial, Social, Spiritual, Occupational, Physical, Intellectual, and Environmental.
Qualifications:
Minimum of an associates Degree in a Human Services related field is required MHRT-C certification or MHRT-C Provisional certification is required.
Personal vehicle use-required.
Responsibilities & Expectations:
Establish Compassionate therapeutic relationships with clients and prospective clients demonstrating the Integrity of AMHC Perform Collaborative case management functions, linking, coordinating, and managing the delivery of services to clients with Excellence Provide Respectful , supportive counseling and assisting clients in selecting and weighing alternatives and making a decision with a focus on Service for the client.

This is a 40-Hour per week position PI226617082 Recommended Skills Licensed Clinical Social Worker (Lcsw) Behavioral Health Case Management Social Work Utilization Management Clinical Supervision Estimated Salary: $20 to $28 per hour based on qualifications.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
